Personal Medication Sheet

During the first important minutes of a medical emergency it is very helpful for Spokane County Fire District 8 Paramedics and Emergency Medical Technicians to know about a patient's medical history, any allergies they may have, and what medications they are currently taking. Sometimes our patient is unconscious or there is no one close by who knows their medical history. A completed Personal Medication Sheet will speak for our patient if needed, if they are confused, unconscious, in pain, or afraid.

We recommend everyone attach a completed Personal Medication Sheet to their refrigerator door. It will tell first responders about your medical conditions, the names of medications you are taking, and who you wish to be contacted in the event of an emergency.

Please take a moment today to complete this Personal Medication Sheet and attach it to your refrigerator door.
